| If we have determined that we have a therapist for your child we need to have you complete the Horizon Intake Packet prior to beginning therapy services. The Developmental History and Patient Profile will be asking you questions about your child that will help your therapist with getting to know more about him or her. We also need the Insurance Form filled out and returned to us by email or mail to 551 South Higley Road, Mesa, Arizona 85206. |

| If your child has qualified for services through the Division of Developmental Disabilities and we have an available therapist you can also contact your state support coordinator to request authorization to begin therapy services with Horizon Pediatric Therapy, Inc. |

| Horizon Pediatric Therapy offers
Physical, Speech, Occupational and Music Therapy. If you are looking for a therapist please take a few minutes to fill out this form. After it is completed it will be forwarded to our office and we will review the information. If we feel we may have a therapist available for you, you will be notified. Each of our therapists have different areas of expertise and availability. Schedules and openings do change frequently. If we do not have an available therapist we will keep your child's information on file for two months. If you are still looking for a therapist after two months, please fill out this form again. |
